- Enrollment fee: $125 per employee (one-time fee)
- Monthly membership fee: $76.88 per month per employee
5+ Employees
- Enrollment fee: $100 per employee (one-time fee)
- Monthly membership fee: $66.88 per month per employee

Same day appointments
We prioritize ease of access, offering same-day in-office or telehealth appointments to help employees get the care they need without missing work. Members also have access to their provider after hours through a dedicated phone line, ensuring they receive guidance whenever they need it.

- Unrushed 30 minute – 1 hour appointment times, these can be longer or shorter based on patient needs

- In office testing to include Rapid COVID, Rapid Flu, Rapid Strep testing and urinalysis evaluation at no additional charge
- In office procedures at no additional charge to include sutures, wart and skin tag removal
- Direct access to your provider via the members only phone number for after hours or weekend emergencies
- Affordable lab options from Quest Diagnostics. Annual labs for men and women range from $25-$100 based on patient needs.
**If a member has a traditional health insurance plan, they can still use this for medications prescribed outside of our office, labs, imaging, referrals or any testing needed outside of our office. Health insurance does not cover the membership fee, or any services provided in our office that is not included in your membership. Any additional fees outside of a membership would be discussed with the patient before services are rendered, some examples of this would include weight loss medications or supplements purchased from our office and hormone replacement therapy.**
